Reformat use-case section and add a table

This commit reformats the Use Case section to:

* define classes of Use Case Risk Factors
* demonstrate several common use cases
* add a table mapping common use cases to forseeable risks

This is still a draft; the table is incomplete but sufficiently demonstrative.
