Clause 5/6: Rewrite to HAS comments/skeleton instructions

Implement HAS comments and skeleton instructions that say:

* We can't tell the manufacturer to write documentation
* Need more guidance and specific examples
* Need more specificity in the Evidence for assessments

Also write provision of methods for operational environment to provide
security functions, especially for integrators, and merge or remove
some duplicate requirements.
